Abstract Title:
The Missing Link in Musculoskeletal Pain: Breathing, Posture, and Nervous System Regulation
Biography:
Alba Valero is a physiotherapist and osteopath with more than 17 years of clinical experience in musculoskeletal, sports, trauma, neurological and rehabilitation settings. She has worked in Barcelona and Dubai and has experience training healthcare professionals nationally and internationally as an INDIBA trainer. Her clinical approach integrates physiotherapy, osteopathy, movement, breathing awareness and patient education. She is the author of “Oxy, el guardián de la respiración consciente”, a book exploring the relationship between breathing, the body and the nervous system.

Abstract:
Musculoskeletal pain is often approached according to the anatomical region where symptoms are experienced. However, breathing, posture, movement, and nervous system regulation are closely interconnected and may influence how the body responds to physical and emotional demands. This presentation proposes an integrated physiotherapy perspective in which breathing is considered not only a respiratory function, but also a component of movement and body awareness.
Clinical observation suggests that altered breathing patterns may coexist with increased muscular tension, reduced thoracic mobility, postural adaptations, and persistent pain. The diaphragm, rib cage, cervical and mandibular regions, pelvic floor, and lumbopelvic system participate in this dynamic relationship, while the nervous system continuously modulates tone, movement, and the perception of threat. Rather than focusing exclusively on the painful area, assessment can therefore include breathing pattern, posture, mobility, muscle tone, and the individual's capacity to regulate and adapt.
The presentation will describe a practical framework for incorporating conscious breathing, gentle movement, postural awareness, and progressive exercise into musculoskeletal physiotherapy. The aim is not to promote a 'perfect' breathing pattern, but to help patients develop greater awareness, adaptability, and confidence in movement. This approach may provide clinicians with an additional perspective for understanding complex musculoskeletal presentations and for supporting patient-centred rehabilitation.
